This feature is most associated with the Orrish (mostly Orks). It is a procedure where they tear their vocal cords and swallow the bleeding for weeks (constant vomiting and sickness) until their voice is hoarse all the time. They gain Disadvantage on any spoken communication checks, but the roar sounds like a demonic command that is highly intimidating. It also creates a vibration the Ducateon hate.

Menacing + Aggression trait (always get full movement, regardless of injuries).
Ducateon Intimidation: If the possessor is Orrish, causes them to make a DC 8 Constitution save or lose Initiative – changes to just after the Ork’s or stays the same if worse.
Glorified Kill: whenever wielder kills an opponent, can use Bonus Action to Intimidate a foe within 20′ who witnessed it. Demoralize one humanoid you can see within 30 feet of you that can see and hear you. Make a Charisma (Intimidation) check contested by the target’s Wisdom (Insight) check. If your check succeeds, the target is frightened until the end of your next turn. If your check fails, the target can’t be frightened by you in this way for 1 hour.
